What is Glaucoma?

Glaucoma is a group of eye diseases that damage the optic nerve, which carries visual information from the eye to the brain. This condition is often called the "silent thief of sight," as it typically develops gradually and without noticeable symptoms until significant vision loss has occurred.

The condition is primarily caused by increased pressure within the eye (intraocular pressure), though it can also develop with normal intraocular pressure.

The optic nerve consists of over one million nerve fibers that transmit visual signals to the brain. When these fibers become damaged due to glaucoma, permanent vision loss occurs, starting with peripheral vision and potentially progressing to complete blindness if left untreated. This makes early detection and treatment crucial for preserving sight.

Types of Glaucoma

Primary Open-Angle Glaucoma

This is the most common form of glaucoma and accounts for the majority of cases. In this type, the drainage angle where the iris meets the cornea appears open, but the drainage system doesn't work properly, causing gradual pressure buildup. It develops slowly and painlessly, making regular eye exams essential for early detection.

Angle-Closure Glaucoma

This type occurs when the iris blocks the drainage angle, preventing fluid from leaving the eye. It can be acute (sudden onset) or chronic (gradual development). Acute angle-closure glaucoma is a medical emergency requiring immediate treatment to prevent permanent vision loss.

Normal-Tension Glaucoma

Also called low-tension glaucoma, this condition involves optic nerve damage despite normal eye pressure. The exact cause hasn’t been determined.

Secondary Glaucoma

This develops as a result of another eye condition or medical issue, such as eye injury, certain medications (particularly steroids), or other eye diseases. Treatment focuses on addressing both the underlying cause and the elevated eye pressure.

Congenital Glaucoma

A rare form present at birth, caused by abnormal development of the eye's drainage system. It requires prompt surgical treatment and careful long-term management to preserve vision and normal eye development.

While we prioritize interventional solutions, we recognize that medications remain an important option for certain patients, particularly in early-stage disease.

Modern glaucoma medications offer improved effectiveness and fewer side effects than older formulations. These medications work by either reducing fluid production in the eye or improving drainage. Many patients can effectively manage their glaucoma with daily eye drop regimens.

However, for those experiencing compliance challenges, side effects, or inadequate pressure control with drops alone, we often recommend transitioning to our interventional approaches for more sustainable results.

The iDose is a tiny implant, placed in the eye during a brief procedure, that continuously releases medication for months, eliminating the need for daily eye drops.

MIGS procedures like iStent, Hydrus, and TrabEX PRO offer effective pressure reduction with minimal tissue disruption and faster recovery times compared to traditional surgery.

The iStent is the world's smallest medical device, creating a permanent opening to improve fluid drainage, while the Hydrus microstent provides similar benefits with a different mechanism suitable for various glaucoma types.

TrabEX PRO is an FDA-approved goniotomy procedure that creates an opening into Schlemm's canal to lower eye pressure without requiring device implantation.

Advanced laser procedures like MP3 TS CPC (micropulse transscleral cyclophotocoagulation) offer precise treatment with reduced complications. These minimally invasive procedures can effectively lower eye pressure while preserving surrounding tissue. Laser treatments often serve as an intermediate step between medications and more invasive surgeries.

For patients requiring more aggressive treatment, we offer cutting-edge procedures, including the Express Mini Shunt and Xen Gel Stent. The Express Mini Shunt creates a controlled pathway for fluid drainage, while the Xen Gel Stent is a soft, flexible tube that maintains drainage with excellent biocompatibility. These newer techniques offer improved success rates with fewer complications than traditional filtering surgeries.

When other treatments aren't sufficient, trabeculectomy and tube shunt procedures remain important options. These time-tested surgeries create new drainage pathways to reduce eye pressure effectively. While more invasive, they offer excellent long-term pressure control for advanced cases.
